Utah State Office of Rehabilitation

The Utah State Office of Rehabilitation (USOR) assists eligible individuals with disabilities to prepare for and obtain employment, and increase their independence. Within the USOR Division of Rehabilitation Services, the Vocational Rehabilitation (VR) program exists to assist and empower eligible individuals with disabilities to achieve and maintain meaningful employment.

Vocational Rehabilitation services can include counseling and guidance, medical services and treatment, assistive technology, training and education, job placement, and follow up services.

To be eligible for VR, the individual

  • Must have a disability
  • The disability causes difficulty in obtaining or maintain a job
  • Requires VR training or services to obtain or maintain a job

To apply for VR, an individual must

  • Fill out an application
  • Make an appointment to see a counselor at a local VR office

You can download a copy of the USOR application form (doc) (pdf). Fill it out completely, sign it, then call the USOR office nearest you (doc) (pdf) to make an appointment for an orientation.
