Summary of Academic Achievement and Functional Performance

IDEA 2004 requires you be given a Summary of Performance (doc) when you leave school by graduating with a diploma or reaching age 22. The summary is useful when it is linked with the IEP process. You will have a chance to take part in the development of this form. It is a written report of what you are able to do in school—your skills in academics and functional abilities. The summary must include recommendations to help you in meeting your post-high school goals.

This report is good information for you, but does not mean you will automatically qualify for any services as an adult.
