Transition
Starting in high school there will be some changes to your IEP (Individual Education Program). The biggest change with be transition. You will decide your dreams and goals and the ways to achieve them. To help you do this, make sure you bring your notebook/ portfolio to your IEP meeting. Your notebook should include your SEOP, transcripts, reports cards, testing and assessments results, certificates and accomplishments. You want to transition to a whole world of possibilities.
Transition is your plan for whole life, not just the single parts. The parts include education, employment, independent living. All together, they will be your life.
Your academic and transition goals do not have to be separate. Think about your classes and transition goals together. There are several tools to help you consider how to make this work. See the toolkit on the left side of this page. There you will find many resources to help you plan your high school career and your transition to adult life. You’ll also find resources you can share with your parents and teachers.
